BREAKING NEWS: Rhapsido (Remibrutinib) FDA Approved for Chronic Hives & Hope for Sjogren's!

October 29, 20253 min read

For years, those of us navigating the world of Chronic Urticaria –– the itching, rashes, swelling and side effects from all the antihistamines.

As a board-certified allergist/immunologist, lifestyle medicine physician, and certified life coach, and as someone who lives with chronic illness (Sjogren’s Disease and dysautonomia), I am thrilled to share some incredibly exciting and long-awaited news for anyone who’s struggled with chronic spontaneous urticaria (CSU), or what many of us simply call chronic hives.

Rhapsido (Remibrutinib): A New FDA-Approved Treatment for Chronic Hives

The FDA recently approved a new medication from Novartis called remibrutinib, marketed as Rhapsido. This is a huge development, and here's why:

Rhapsido is a game-changer for chronic hives because it is the first-of-its-kind oral treatment known as a BTK inhibitor. (No, we’re not talking about the K-pop band- thats BTS! 😉)

The Science: How a BTK Inhibitor Works

BTK, or Bruton's tyrosine kinase, is an enzyme inside your immune system's cells, particularly your mast cells and B-cells. It acts like an "on switch" for inflammation. When this switch is flipped, it leads to the release of histamine and other chemicals that cause those itchy, frustrating hives.

Remibrutinib works by blocking this BTK switch, essentially telling mast cells to calm down and not release histamine. Targeting this specific pathway offers a highly effective and precise way to control hives right at the source. It’s not just a band-aid; it's working on a deeper level to prevent the reaction from happening in the first place.

The Sjogren's & Autoimmune Connection: Why This Is SO Exciting

While this news is fantastic for the millions living with chronic hives, I'm personally even more excited about what it could mean for patients bridging the allergy and autoimmune spectrum – including those of us with Sjogren's Disease and related conditions!

Here's why this is relevant:

  • B-Cells and Inflammation: We know that B-cells and other immune pathways involving BTK are major players in the inflammation and damage that occurs in Sjogren's.

  • Overlap with Mast Cell Activation: I have many patients who have an overlap between Sjogren's Disease and misbehaving mast cells, often presenting with chronic hives or other symptoms suggestive of mast cell activation.

  • Beacon of Hope: The fact that remibrutinib has been proven safe and effective for another chronic inflammatory condition and received FDA approval is incredibly hopeful. It shows that targeting this specific part of the immune system is a viable and powerful strategy that may be applicable to other conditions.

For those of us in the autoimmune community, this isn't just news about a different condition; it is a beacon of hope. It’s a sign that science is moving forward, researchers are understanding complex immune pathways better, and new targeted treatments for us are getting closer and closer.

Cautious Optimism: What Happens Next?

Looking ahead, I have cautious optimism. The journey for Sjogren's drug approval is still ongoing, and research takes time. However, today's news is a significant and powerful step in the right direction for a possible medication that may help people living with multiple conditions that bridge that allergy-autoimmune spectrum.

It's a win for patients, for science, and a huge boost of hope for everyone living with chronic immune-mediated conditions. I will be following upcoming trials for more medications in the pipeline for Sjogren's Disease and mast cell-related disorders. We'll be sure to keep you updated as we learn more!
